WAIVER OF LIABILITY: I hereby wish to participate or have my child participate in the above named sailing program conducted by the Chautauqua Lake Community Sailing Foundation (hereafter referred to as CLCSF).  I recognize and understand that this program is voluntary in nature, requires that the participant certify their ability to swim and that participation is at all times at the discretion of CLCSF.  I recognize that the participants of this program incur risks attendant to sailing and that this activity may involve maneuvering a boat, or watercraft over deep waters in potentially hazardous conditions which may include among other things, cold water temperature, strong winds and high waves, sudden and unexpected immersion in deep waters and collision with other watercraft or stationary objects such as docks, pilings, and buoys.  I further recognize that the risks involved with these activities may result in serious injury or death.  I understand that I am responsible for my actions or the actions of my child while I or  he or she may be participating in this program, and I fully agree to waive any and all claims, charges, losses and liabilities including those caused by negligence, against CLCSF, their respective officers, board members, and against any and all volunteers, parents, participants, or others, which may arise from, or in any way be in connection with, the practices or activities of this sailing program.  I also agree to be bound by the racing rules of the ISAF, US SAILING, State and Federal laws concerning watercraft, and by any other stated or otherwise written rules which govern this sailing program.

Covid Waiver

Please read and sign the statement below regarding Covid-19. This form requires signatures from parents/guardians and return copy to Sailing Director prior to your child’s attendance in the camp. Participating in Chautauqua Lake Community Sailing Foundation Camp may place your child/children/family at risk of having contact with individuals who may have been exposed to and/or have been diagnosed with one or more communicable diseases, including but not limited to Covid-19 or other contagious medical conditions, diseases or maladies. I/we understand it is impossible to eliminate the risk and the exposure to and/or the infection through contact with or close proximity to an individual with a communicable disease, and I/we assume this risk. I Have read the above statement regarding COVID-19 and other communicable diseases. I/We understand and agree to the policies established by the Chautauqua Lake Community Sailing Foundation for the safety of all participants related to COVID-19 and other communicable diseases as they may be made publicly known.
